Integrated Delivery System

A plan to develop and coordinate the Integrated Delivery System (IDS) was commissioned by the Community Care Collaborative board in September 2014. The IDS is intended to coordinate the work of Travis County’s health care providers in serving the safety net population. The IDS plan is designed to accomplish the following:

  • Develop new models of care coordination that connects patients to a spectrum of services and providers, all working in collaboration with each other
  • Improve health information technology so providers are equipped with the tools to access and share up-to-date health care data
  • Create measurement and assessment tools that will allow the CCC and its partners to develop best practices and improve health care services
  • Expand select specialty care services to allow for more timely and efficient access to care

The relationship between Central Health, Seton, and the University of Texas leads to person-centered, high-quality, cost-efficient care.

The CCC’s plan to develop and implement the IDS will take several years and require the coordination of hospitals, physicians, social services and health plan providers.
